issue #13 (IK8MWS) — LSP 集成 · TypeScript Language Server 接入

新增 `src/services/lsp/`(5 模块)+ `src/tools/builtin/`(3 工具),把 typescript-language-server 接入 alice-cli 的工具系统:
- **lspGotoDefinition**: 跳转到符号定义位置
- **lspFindReferences**: 查找符号所有引用
- **lspDocumentSymbol**: 返回文件顶层 exports

| tsls 探测 | `command-exists.sync` + 进程内缓存;缺失抛 `LspNotInstalledError` + `TSLS_INSTALL_HINT` |
| 单例 | lazy + single-flight `inflight` promise,避免并发启动泄漏 subprocess |
| Location[] 序列化 | 按 file path 分组并发读盘,消除 N+1 stat(50 references → 1 readFile) |
| 工具错误边界 | 共享 `withLspToolErrorBoundary`,统一 install hint / 透传其它错误 |

## 不动的东西

- `src/shim/qwen-code-core.ts` 的 `NativeLspClient` / `NativeLspService` 空壳(死代码)保持原样,本 PR 不触碰
- `--experimental-lsp` flag 行为不变(CLI config 仍按 flag 启停 LSP,本 PR 是底层实现)

## 风险点

- macOS `command-exists` 在 PATH 为空时会回退到 `confstr(_CS_PATH)`,所以 "PATH 隔离测缺失" 改用绝对不存在的 binary 名替代
- Bun 的 `Subprocess.kill` 是 this-bound,不能安全剥离成普通方法;改用 `process.kill(pid, signal)` 跨 runtime 通用
- tsls 启动需要 `npm i -g typescript-language-server`,缺失时工具返回安装提示而非崩溃

## 后续 P1

- 实现 LSP workspace/symbol / completion / hover(本 PR 仅做基础三件套)
- documentSymbol flatten 移到独立的 `symbolFormat.ts`,复用 LSPClient.symbol 处理
- Location[] 序列化的 textCache 加 LRU 限定大小(目前无界,长会话内存增长)
