### **Reason**
FixAddMissingNewOperator was implemented before the unified CodeFixProvider and stable diagnostic codes, relying on ad-hoc wiring and non-ETS snippets.

### **Description**
This refactor migrates the fix to the current model by using generated fix IDs and supported diagnostics, routing tests through GetCodeFixesAtPositionImpl, and ensuring applicability only for class constructors (skipping plain functions or existing new calls). Test sources are updated to ETS-compliant code, and assertions normalized to check for correct replacements (Foo() → new Foo()) or no actions where appropriate.
